What's Included
A review of your personal and family medical history, current medications and supplements, and your list of healthcare providers. You'll complete a health risk assessment covering diet, exercise, tobacco and alcohol use, mental health, and fall/home safety, plus basic measurements like height, weight, BMI, and blood pressure. For older adults, we also perform a cognitive assessment. We close the visit with a discussion of recommended screenings, immunizations, and a personalized prevention plan.
